Compensation: $58,000-$65,000 annually
About the Role
We are seeking a compassionate, highly organized Patient & Family Support Coordinator to support families and caregivers as they begin using a healthcare technology device designed to improve communication and daily independence. This role plays a critical part in onboarding, education, care coordination, and ongoing support-helping families feel confident, informed, and successful.
This is a newly created, growing role, ideal for someone who thrives in evolving environments, enjoys working directly with patients and families, and is passionate about healthcare support, patient experience, and service excellence.
Key Responsibilities
- Provide onboarding and ongoing support to patients, caregivers, and families through phone, email, and virtual communication
- Guide users through device setup, basic functionality, daily use, modeling strategies, and habit-building
- Serve as a trusted point of contact, offering reassurance, education, and problem-solving support
- Assist families with understanding healthcare program or insurance-related requirements, documentation, timelines, and usage expectations
- Accurately document patient interactions, case notes, and outcomes in CRM or case management systems
- Monitor engagement and escalate complex clinical, technical, or insurance-related needs to specialized internal teams
- Collaborate cross-functionally to ensure a seamless, patient-centered experience
- Provide coverage and support to team members as needed
- Maintain strict confidentiality and compliance with HIPAA and healthcare privacy regulations
Qualifications
- 2+ years of experience in healthcare customer service, patient support, care coordination, patient intake, medical customer support, or health services administration
- Experience working directly with patients, families, or caregivers in a healthcare, medical device, insurance, or clinical support environment
- Strong empathy, emotional intelligence, and ability to support individuals during stressful or emotional situations
- Excellent verbal and written communication skills
- Highly organized with strong documentation and time management skills
- Comfortable working independently in a remote environment
- Proficient with computers and able to learn new systems and workflows
- Experience with CRM platforms such as Salesforce, electronic health records (EHR), or case management systems is highly preferred
- Bachelor's degree preferred, but equivalent experience will be considered
